SHS PTSA TAX-EXEMPT FORM
This form should be used when purchasing items on behalf of SHS PTSA. As a non-profit entity, SHS PTSA does not pay sales tax on items used for the school, and therefore cannot reimburse any sales tax paid on items bought by volunteers.

REIMBURSEMENT / CHECK REQUEST
Follow the instructions on the form to request reimbursement from PTSA, or to request payment of an invoice received from a vendor.

*Remember sales tax is not reimbursed.  SHS PTSA is a registered non-profit 501 (c)(3) organization.

Use this form when depositing money on behalf of SHS PTSA.
